Specialist Support Coordination- NDIS
Specialist Support Coordination is designed for NDIS participants with more complex needs who require a higher level of coordination, clinical insight and risk management. It goes beyond linking services — it provides structured, therapeutic oversight to help stabilise supports, reduce risk and build long-term capacity.
About
Create Change Counselling is led by Chloe Flemming, an Accredited Mental Health Social Worker and Accredited Clinical Social Worker (AASW: 507632)
Throughout my career, I have worked alongside individuals facing layered presentations — where anxiety, trauma, disability, life stress and systemic pressures intersect. This experience has shaped a grounded, trauma-informed approach that recognises the broader context of a person’s life, not just their symptoms.
Social work brings a holistic lens to therapy. It means understanding how family roles, workplace expectations, health systems, cultural context and structural barriers can all influence wellbeing. It also means recognising strengths, resilience and the capacity for change — even when circumstances feel overwhelming.
In private practice, I integrate this broader perspective with evidence-based therapeutic approaches, including EMDR and cognitive and behavioural therapies. My aim is to offer thoughtful, collaborative support that is both clinically informed and practically grounded.
What to Expect in Counselling
Starting therapy can feel like a big step. It’s common to feel unsure, nervous or even hesitant before your first appointment. My role is to create a steady, respectful space where you can begin at your own pace.
Your First Session
Our first session is an opportunity to understand what has brought you to counselling and what you would like to be different. We’ll talk about your current concerns, relevant history, and your goals for therapy. You don’t need to have everything clearly articulated — we can make sense of it together.
There is no pressure to share more than feels comfortable. Safety and trust develop over time.
A Collaborative and Individualised Approach
Therapy is not about being “fixed.” It’s about understanding patterns, strengthening coping capacity, and creating meaningful change.
My approach is trauma-informed and evidence-based. Depending on your needs, this may include:
EMDR for trauma processing
Cognitive and behavioural strategies for anxiety
Mindfulness and grounding skills
Exploring role conflict, stress and life transitions
Strengthening boundaries and self-compassion
For clients who are neurodivergent or living with disability, therapy is not aimed at changing who you are. Instead, we focus on reducing distress, navigating barriers, and building practical supports that honour your strengths, needs and lived experience.
Therapy is always tailored to you. We move at a pace that feels manageable and respectful of your nervous system.
