Create Change Counselling is run by Chloe Flemming, Accredited Clinical Social Worker and Accredited Mental Health Social Worker (AASW 507632).

Chloe studied at the University of Sydney and has over 20 years experience supporting people navigating complex life situations, trauma and systemic barriers to care in Australia and the UK prior to opening Create Change Counselling.

I work in a way that is non judgemental, inclusive and attentive to the broader life contexts that shape your experience. I integrate this wider perspective with trauma informed, evidence based therapies including EMDR, CBT, Motivational interviewing, Narrative therapy, Interpersonal therapy and mindfulness training.

Practical Details

Sessions run for 50–60 minutes and can be held in person in Perth or via secure online video.

If you have a Mental Health Care Plan from your GP, Medicare rebates may apply. Private health rebates may also be available depending on your fund.

If you’re unsure whether counselling is right for you, you’re welcome to arrange a brief introductory call to ask questions before booking. 

Fees & Medicare Mental Health Care Plans

Seeking support is an investment in your wellbeing, and it’s important to have clear information about costs from the outset.

Session Fees

Initial consult: $190

Standard sessions: $160

Concession: $100

Generally Fees are payable at the time of your appointment with rebates available for eligible people. Bulk Billing is also available for those who need it.

25% of our availability is dedicated bulk billing with Health Care Card holders and other concessions prioritised for these slots. 

If you need to cancel, please provide 24 hours notice or you may need to pay the full fee. Just reply No to our reminder text and it deletes the appointment.

Medicare Mental Health Care Plans

If you have a valid Mental Health Care Plan from your GP, you may be eligible for a Medicare rebate for a set number of sessions each calendar year.

With a Mental Health Care Plan:

  • You pay the full session fee upfront (except people we have agreed to bulk bill Medicare for).

  • Medicare rebates are processed after payment.

  • A referral letter from your GP is required prior to your first session under the plan.

If you’re unsure how to obtain a Mental Health Care Plan, your GP can discuss this with you and provide the necessary referral if appropriate.

Private Health Insurance

Depending on your level of cover, you may be able to claim a rebate through your private health fund. Rebates vary between providers and policies, so it’s best to confirm your entitlement directly with your insurer.

NDIS Participants:

Assessment, Therapy, Training and support coordination services are available. All fees are within the NDIA guidelines for 2025-2026 and updated accordingly. We are a registered provider of social work services including specialist support coordination, assessment, training and therapy.
